If you participate with an HMO and are referred to a specialist, all you need to call our managed care department 24/7 with your appointment date.

You may leave your message on the referral voice mail including your name with spelling, date of birth, name of specialist you will be seeing, location of the office, the appointment date, the type of insurance you have, and a daytime phone number. Please allow at least 5 days to complete your referral.
